چکیده

Background/Introduction Lung isolation, a technique largely used to facilitate access during thoracic surgery, can create some difficulty in maintaining a patient’s blood gas balance. Strategies have been used to overcome the ventilation-perfusion mismatch associated with one-lung ventilation (OLV). However, such strategies may induce volutrauma, barotrauma and atelectotrauma in the ventilated lung, leading to acute lung injury (ALI) and/or acute respiratory distress syndrome (ARDS). Different ventilatory parameters have been used to improve the safety and efficacy of OLV, with the use of high or low tidal volumes (VT) being the most contentious.

Acute lung injury in thoracic surgery.

PURPOSE OF REVIEW This review will analyze the risk factors of acute lung injury (ALI) in patients undergoing thoracic surgery. Evidence for the occurrence of lung injury following mechanical ventilation and one-lung ventilation (OLV) and the strategies to avoid it will also be discussed. RECENT FINDINGS Post-thoracotomy ALI has become one of the leading causes of operative death. The pathoge...

Small tidal volumes, positive end-expiratory pressure, and lung recruitment maneuvers during anesthesia: good or bad?

Anesthesiology, V 123 • No 3 501 September 2015 M ECHANICAL ventilation may cause harm to the lung, at least if the lung is already hurt as is the case in intensive care. Can it be harmful also when ventilating the “lung-healthy” patient during anesthesia? Probably not during anesthesia alone, as suggested by animal experiments on prolonged mechanical ventilation,1,2 but inflammatory events,3 s...

Aprospective randomized study comparing respiratory efficacy of percutaneous transtracheal and endotracheal, low-frequency jet ventilation during microlaryngeal surgery

Abstract Background: Transtracheal jet ventilation is an alternative ventilatory approach in airway surgery. The purpose of this study was to compare respiratory efficacy of percutaneous transtracheal with endotracheal, low frequency jet ventilation during microlaryngeal surgery. Methods: Ninety ASA class I-II patients aged between 16 and 70 years undergoing elective microlaryngeal surgery wer...

Comparison of two protective lung ventilatory regimes on oxygenation during one-lung ventilation: a randomized controlled trial

BACKGROUND The efficacy of protective ventilation in acute lung injury has validated its use in the operating room for patients undergoing thoracic surgery with one-lung ventilation (OLV). The purpose of this study was to investigate the effects of two different modes of ventilation using low tidal volumes: pressure controlled ventilation (PCV) vs. volume controlled ventilation (VCV) on oxygena...

تأثیر تهویه با حجم‌های جاری متفاوت و تهویه دقیقه‌ای ثابت بر روی دی‌اکسید کربن انتهای بازدمی

Background & Aims: Elimination of carbon dioxide is related to the alveolar ventilation, respiratory rate and tidal volume. The aim of this study  to determine the effect of mechanical ventilation with different tidal volumes and constant minute ventilation on the end tidal Co2  and oxygen saturation during general anesthesia and to seek optimum parameters of mechanical ventilation during gener...
